ABSTRACT

Shoulder fracture is one of the most frequently treated injuries in trauma centers, with an overall incidence that appears to have increased in recent years, ranging from 219 to 419 cases per 100 000 person-years. In clinical terms, shoulder girdle injury is difficult to diagnose due to the close relationship between the shoulder and the chest, and imaging identification of the different types of injuries can be challenging. In this context, X-rays are the most appropriate method and the cornerstone of the initial approach to shoulder trauma, and at least 3 views are recommended: true anteroposterior view (AP), axial or axillary projection or modified axial projection (Velpeau view), and lateral scapula shoulder or Y view. However, patient positioning is often problematic due to the additional pain associated with limb mobilization in order to achieve the proper position for radiographic projection. The following is the description of a technique for performing an axial shoulder projection that is free of these complications, easy to standardize, and applicable to any traumatic or degenerative disease of the proximal humerus or glenohumeral joint, which, to the best of the authors' knowledge, has not been previously published.


El trauma de hombro es una de las patologías más frecuentemente vista en los centros de trauma con una incidencia global que aparentemente ha aumentado en los últimos años y que se ha reportado entre 219 a 419 casos por 100000 personas-año. El diagnóstico del trauma de la cintura escapular no es fácil clínicamente por la íntima relación entre el hombro y el tórax, y la identificación imagenológica de las distintas lesiones puede ser desafiante. Las radiografías son el método más apropiado y piedra angular del estudio inicial en el trauma de hombro, el set de trauma tiene al menos 3 planos: la vista Anteroposterior (AP) verdadera, una proyección axial o axilar o su modificación descrita por Velpeau. Y una proyección Y de escápula. Sin embargo, el posicionamiento del paciente muchas veces no es fácil por el dolor adicional que puede generar la movilización de la extremidad para lograr la posición adecuada para la proyección radiográfica. Describimos una técnica para realizar una proyección axial de hombro sin estas dificultades, fácilmente estandarizable para cualquier patología traumática o degenerativa del húmero proximal o de la articulación gleno-humeral y que creemos no ha sido publicada previamente.

ABSTRACT

RESUMEN En muchas ocasiones, los pacientes con tumores cerebrales tienen una variedad de síntomas psiquiátricos inespecíficos. Algunos de estos pueden constituir la primera o única manifestación del tumor, sin la presencia de ningún síntoma o signo neurológico. El diagnóstico ha de basarse en la anamnesis completa y en la exploración neurológica; la neuroimagen confirmará el diagnóstico clínico. Con el presente trabajo se describió la asociación inusual de hematoma subdural crónico y meningioma parasagital en un caso presentado. Se trató de un paciente de 68 años con antecedentes de esquizofrenia. Acudió a consulta con una hemiparesia izquierda. Se le realizó una tomografía axial computarizada de cráneo y arrojó un hematoma subdural crónico y un meningioma parasagital derecho. Se le aplicó tratamiento quirúrgico en dos tiempos operatorios. Su evolución posquirúrgica transcurrió sin complicaciones (AU).


ABSTRACT In many cases, patients with brain tumors have a variety of nonspecific psychiatric symptoms. Some of them can be the first or the only manifestation of the tumor, without presenting any neurological signs or symptoms. The diagnosis must be based on the complete anamnesis and on the neurological examination. The neuroimaging will confirm the clinical diagnosis. The unusual association of a chronic subdural hematoma and a parasagittal meningioma was described in a case presented in the current work. It dealt with a patient, aged 68 years with antecedents of schizophrenia. He assisted the consultation with left hemiparesis. A skull computed tomography showed a chronic subdural hematoma and a right parasagittal meningioma. He underwent a two-steps surgery. His post-surgery evolution ran without complications (AU).

ABSTRACT

RESUMEN El absceso cerebral es un proceso infeccioso focal del parénquima cerebral. Se inicia con un área localizada de cerebritis y progresa a una colección de pus rodeada por una cápsula bien vascularizada. La mortalidad oscila entre 5 a 15 % de los casos, excepto en la ruptura intraventricular del absceso cerebral, situación en que la mortalidad oscila entre 38 a 84 %, con tasas altas de discapacidad en los sobrevivientes. Se presentó un caso de 47 años, con sintomatología neurológica infecciosa, además de signos neurológicos que demuestran el trastorno funcional del lóbulo temporal no dominante. Se realizaron varios exámenes complementarios y se diagnosticó dos abscesos cerebrales temporales derechos. Fue intervenido neuroquirúrgicamente, su evolución fue satisfactoria con regresión de casi la totalidad de los síntomas prequirúrgicos presentados (AU).


ABSTRACT Brain abscess is a focal infectious process of the brain parenchyma. It begins with a located area of cerebritis and progresses to a pus collection surrounded by a well-vasculirized capsule. Mortality oscillates from 5 % to 15% of the cases, except in the intraventricular rupture of the brain abscess, situation in which mortality oscillates from 38 % to 84 %, with high rates of disability in survivors. The case presented is the case of a patient aged 47 years, with infectious neurologic symptoms besides neurologic signs showing the functional disorder of the non-dominant temporal lobe. Several complementary tests were carried out and two right temporal brain abscesses were diagnosed. The patient underwent a neurosurgery; his evolution was satisfactory with the almost total regression of the symptoms before surgery (AU).

ABSTRACT

Resumen Las fracturas del Atlas representan el segundo tipo más común de lesiones de la columna cervical alta. Con este trabajo se persigue describir la fijación occipito-cervical como tratamiento de la fractura de atlas tipo Jefferson inestable. Se presenta paciente que sufre trauma raquimedular, después de aplicarle un minucioso examen físico y la tomografía axial computada de columna cervical se detecta una fractura tipo Jefferson de atlas con más de 7 mm de separación de sus fragmentos. Al ser esta una fractura inestable de la región cervical alta se decide realizar una fijación occipito-cervical como método de tratamiento de esta lesión. La evolución postquirúrgica del paciente transcurrió sin complicaciones (AU).


Summary Atlas fractures represent the second most common type of upper cervical spine injury. This work aims to describe the occipito-cervical fixation as a treatment for the unstable Jefferson-type atlas fracture. A patient suffering from spinal cord trauma is presented, after applying a thorough physical examination and the computed tomography of the cervical spine, a Jefferson type fracture of atlas with more than 7 mm of separation of its fragments is detected. As this is an unstable fracture of the upper cervical region, it was decided to perform an occipito-cervical fixation as a treatment method for this lesion. The postoperative evolution of the patient was uneventful (AU).

Abstract Background and objectives: The perioperative cardiac arrest (CA) and mortality rates in Brazil, a developing country, are higher than in developed countries. The hypothesis of this review was that knowledge of the epidemiology of perioperative CA and mortality in Brazil enables the comparison with developed countries. The systematic review aimed to verify, in studies conducted in Brazil, the epidemiology of perioperative CA and mortality. Method and results: A search strategy was carried out on different databases (PubMed, EMBASE, SciELO and LILACS) to identify observational studies that reported perioperative CA and/or mortality up to 48 hours postoperatively in Brazil. The primary outcomes were data on epidemiology of perioperative CA and mortality. In 8 Brazilian studies, there was a higher occurrence of perioperative CA and mortality in males; in extremes of age; in patients in worse physical status according to the American Society of Anesthesiologists (ASA); in emergency surgeries; in general anesthesia; and in cardiac, thoracic, vascular, abdominal and neurological surgeries. The patient's disease/condition was the main triggering factor, with sepsis and trauma as the main causes. Conclusions: The epidemiology of both perioperative CA and mortality events reported in Brazilian studies does not show important differences and, in general, is similar to studies in developed countries. However, sepsis represents one of the major causes of perioperative CA and mortality in Brazilian studies, contrasting with studies in developed countries in which sepsis is a secondary cause.


Resumo Justificativa e objetivos: As incidências de parada cardíaca (PC) e de mortalidade perioperatória no Brasil, um país em desenvolvimento, são mais elevadas em relação às dos países desenvolvidos. A hipótese desta revisão é que o conhecimento da epidemiologia de PC e de mortalidade perioperatória no Brasil possibilita sua comparação com a dos países desenvolvidos. A revisão sistemática teve como objetivo verificar, em estudos realizados no Brasil, a epidemiologia de PC e de mortalidade perioperatória. Conteúdo: Realizou-se estratégia de busca em diferentes bases de dados (PubMed, EMBASE, SciELO e LILACS) para a identificação de estudos observacionais que reportaram PC e/ou mortalidade perioperatória até 48 horas pós-operatório no Brasil. Os desfechos primários foram dados de epidemiologia de PC e de mortalidade perioperatória. Em 8 estudos nacionais, identificou- se maior ocorrência de PC e de mortalidade perioperatória no sexo masculino, em extremos de idade, em pacientes em pior estado físico segundo a American Society of Anesthesiologists (ASA), em cirurgias de emergência, em anestesia geral, e em cirurgias cardíaca, torácica, vascular, abdominal e neurológica. A doença/condição do paciente foi o principal fator desencadeante, tendo como causas principais a sepse e o trauma. Conclusões: Nos estudos nacionais, a epidemiologia dos eventos tanto de PC como de mortalidade perioperatória não apresenta diferenças importantes, e de maneira geral, é semelhante à de estudos de países desenvolvidos. Entretanto, a sepse, nos estudos nacionais, representa uma das principais causas de PC e de mortalidade perioperatória, diferenciando-se dos estudos de países desenvolvidos nos quais a sepse é causa secundária.

Abstract In kidney biopsies reviews, scleroderma renal crisis (SRC) is characterized by vascular endothelial injuries, C4d deposits on peritubular vessels, and acute and chronic injuries coexisting on the same biopsy. The clinical signs of thrombotic microangiopathy (TMA) are described in systemic sclerosis (SSc), nevertheless, it has not been related to acute injuries described on kidney biopsies. We report a case of SRC in a patient with scleroderma-dermatomyositis overlap syndrome, which also showed clinical and histopathological data of TMA. On fundus examination, a severe acute hypertensive retinopathy was found. The kidney biopsy showed severe endothelial damage with widening of mucoid cells at the level of the intima, focal concentric proliferation on most small arterioles, and C3, C4d, and IgM deposits along the capillary walls. The genetic study of complement only showed the presence of membrane cofactor protein (MCP) risk haplotypes, without other genetic complement disorders. We understand that in a patient with TMA and SSc, the kidney damage would be fundamentally endothelial and of an acute type; moreover, we would observe clear evidence of complement activation. Once further studies correlate clinical-analytical data with anatomopathological studies, it is likely that we will be forced to redefine the SRC concept, focusing on the relationship between acute endothelial damage and complement activation.


Resumo Nas revisões de biópsias renais, a crise renal esclerodérmica (CRE) é caracterizada por lesões endoteliais vasculares, depósitos de C4d em vasos peritubulares e lesões agudas e crônicas que coexistem na mesma biópsia. Os sinais clínicos de microangiopatia trombótica (MAT) são descritos na esclerose sistêmica (ES); no entanto, não foram relacionados às lesões agudas descritas nas biópsias renais. Relatamos um caso de CRE em um paciente com síndrome de superposição de esclerodermia-dermatomiosite, que também apresentou dados clínicos e histopatológicos de MAT. No exame de fundo do olho, foi encontrada uma retinopatia hipertensiva aguda grave. A biópsia renal mostrou lesão endotelial grave com alargamento das células mucoides ao nível da íntima, proliferação concêntrica focal na maioria das pequenas arteríolas e depósitos de C3, C4d e IgM ao longo das paredes dos capilares. O estudo genético do complemento mostrou apenas a presença de haplótipos de risco da proteína cofator de membrana (PCM), sem outros distúrbios genéticos do complemento. Entendemos que em um paciente com MAT e ES, o dano renal seria fundamentalmente endotelial e do tipo agudo; além disso, observaríamos evidências claras de ativação do complemento. Uma vez que novos estudos correlacionam dados clínico-analíticos com estudos anatomopatológicos, é provável que sejamos forçados a redefinir o conceito de CRE, enfocando a relação entre dano endotelial agudo e ativação do complemento.

ABSTRACT Introduction: Congenital fibrinogen disorders are rare conditions in which there are quantitative and qualitative alterations of factor I; the vast majority of patients are asymptomatic. Case presentation: A 19-year-old female patient with a history of congenital hypofibrinogenemia presented with spontaneous vulvar hematoma along with hypotension, tachycardia, stupor and hematoma of 20cm in the right labium majus. On admission, the young woman had hemoglobin 6.6 g/dL, fibrinogen 74 mg/dL and prolonged clotting times. She received red blood cells transfusion and cryoprecipitates, followed by surgical drainage and intravenous fibrinogen replacement, adjusting the dose according to fibrinogen levels in plasma. The patient presented progressive improvement without hemorrhagic recurrence and fibrinogen levels within the target values until hospital discharge. Discussion: Afibrinogenemia and hypofibrinogenemia are part of the quantitative factor I disorders; in the first case, there is total absence of circulating fibrinogen, and in the second case the levels are below 150 mg/dL. Spontaneous vulvar hematoma as a severe hemorrhagic manifestation is not frequent in symptomatic patients; its treatment is based on fibrinogen replacement in an individualized manner and surgical management when required. Conclusion: Hypofibrinogenemia is a rare disease, and fibrinogen replacement is one of the mainstays of treatment.


RESUMEN Introducción. Los trastornos congénitos del fibrinógeno son una rara condición donde se presentan alteraciones cuantitativas y cualitativas del factor I, siendo asintomáticos la gran mayoría de pacientes. Presentación del caso. Paciente femenino de 19 años con antecedente de hipofibrinogenemia congénita, quien cursa con hematoma espontáneo en vulva y presenta hipotensión, taquicardia, estupor y hematoma de 20cm en labio mayor derecho. Al ingreso, la joven registra hemoglobina 6.6 g/dL, fibrinógeno 74 mg/dL y prolongación de tiempos de coagulación. Se transfunden glóbulos rojos y crioprecipitados; luego se hace drenaje quirúrgico y reposición de fibrinógeno ajustando dosis acorde a fibrinógeno plasmático. La paciente presenta mejoría progresiva sin recurrencia hemorrágica y niveles de fibrinógeno en metas hasta egreso hospitalario. Discusión. La afibrinogenemia y la hipofibrinogenemia hacen parte de los trastornos cuantitativos del factor I, con ausencia total para la primera y niveles <150 mg/dL para la segunda. El hematoma espontáneo de vulva como manifestación hemorrágica severa no es una presentación habitual en pacientes sintomáticos; su tratamiento se basa en la reposición de fibrinógeno de forma individualizada y manejo quirúrgico cuando sea requerido. Conclusión. La hipofibrinogenemia es una enfermedad rara, donde el reemplazo de fibrinógeno es uno de los pilares de tratamiento.

Abstract Introduction "Episodic" memory problems are common in people with cognitive impairment due to Alzheimer's disease and related disorders. Dubois et al. developed the Five-Word Test (5WT) to evaluate episodic memory, which has proved to be an easy and valid test for identifying cognitive disorders. However, its validation and cultural adaptation from French to Spanish has not been undertaken and its usefulness in Mexican population is unknown. Objective Validation and cultural adaptation of the 5WT for screening minor and major neurocognitive disorder (ND) in Mexican older adults with probable Alzheimer's disease. Method Two hundred and fifteen participants (70 cognitively healthy subjects, 73 with minor ND and 72 with major ND were included). The cognitive status (gold standard) was determined using current clinical criteria and neuropsychological evaluation. The Spearman coefficient, ROC curve, and multinomial logistic regression models were used to determine the concurrent validity of the 5WT. Results The correlation between the 5WT and the Mini-Mental State Exam (MMSE) was .58, whereas for the clock face test it was -.37 (p < .001). The area under the 5WT curve was .97 (95% CI [.94, .99]), with a cut-off point of ≤ 16/20 for the diagnosis of major ND (89% sensitivity, 98% specificity) and .77 (95% CI [.70, .85]) for minor ND with a cut-off point of ≤ 18/20 (66% sensitivity, 77% specificity). Discussion and conclusion Since the 5WT is a simple, valid instrument for the identification of neurocognitive disorders like Alzheimer's disease, it could be a practical screening test.


Resumen Introducción Los problemas de la memoria "episódica" son comunes en las personas con deterioro cognitivo tipo Alzheimer. Dubois et al. desarrollaron la Prueba de Cinco Palabras (P5P) para evaluar la memoria episódica, la cual ha demostrado ser sencilla y válida para identificar trastornos cognitivos. Sin embargo, su validación y adaptación cultural del francés al español no se ha realizado y se desconoce su utilidad en población mexicana. Objetivo Validación y adaptación cultural de la P5P para el tamizaje del trastorno neurocognitivo (TNC) menor y mayor en adultos mayores mexicanos con probable enfermedad de Alzheimer. Método Participaron 215 participantes (70 cognitivamente sanos, 73 con TNC menor y 72 con TNC mayor. El estado cognitivo (estándar de oro) se determinó mediante los criterios clínicos vigentes y de evaluación neuropsicológica. El coeficiente de Spearman, la curva ROC y modelos de regresión logística multinomial se utilizaron para determinar la validez concurrente de la P5P. Resultados La correlación entre la P5P y el MMSE fue de .58, mientras que para la prueba de reloj fue de -.37 (p < .001). El área bajo la curva de la P5P fue .97 (IC 95% [.94, .99]), con un punto de corte ≤ 16/20 para el diagnóstico del TNC mayor (sensibilidad: 89%, especificidad: 98%) y de .77 (IC 95% [.70, .85]) para el TNC menor con un punto de corte ≤ 18/20 (sensibilidad: 66%, especificidad: 77%). Discusión y conclusión La P5P es un instrumento válido y simple para identificar de trastornos neurocognitivos de tipo Alzheimer por lo que podría ser una prueba práctica para uso en el tamizaje.

RESUMEN Introducción: los meningiomas constituyen la segunda causa de tumores cerebrales primarios, en el adulto. Representan hasta el 32 % del total de los mismos. Objetivo: describir el comportamiento del meningioma intracraneal en los pacientes investigados. Materiales y métodos: se realizó un estudio analítico, descriptivo, retrospectivo a los pacientes neurointervenidos con meningioma intracraneal, en el Servicio de Neurocirugía del Hospital Docente Universitario "Comandante Faustino Pérez Hernández", de la provincia Matanzas. En el período comprendido entre el 1ero de enero de 2017 al 1ero de enero del 2019. Los 15 pacientes intervenidos conformaron el universo de estudio. Resultados: la media poblacional fue de 55 años. Predominó el sexo femenino en un 73,3 %. La cefalea fue la manifestación clínica más frecuente, igualmente, que los meningiomas de la convexidad cerebral. En cuanto a la topografía, la media del tamaño de la lesión fue de 4,4 cm. La variedad meningotelial (40 %) fue la que predominó. El grado II de resección fue el que más se empleó. El edema cerebral postquirúrgico predominó en un 26,6 %. La puntuación de la escala de Karnofsky al egreso fue superior que al ingreso. Conclusiones: cuanto más precoz se realice el diagnóstico clínico y tratamiento quirúrgico en los pacientes portadores de meningioma, mejor será su calidad de vida al egreso (AU).


ABSTRACT Introduction: meningiomas are the second cause of primary brain tumors in adults, representing up to 32 % of the total. Objective: to describe the behavior of intracranial meningioma in the studied patients. Materials and methods: a retrospective, descriptive, analytical study was conducted on neurosurgery patients with intracranial meningioma in the Neurosurgery service of the University Teaching Hospital "Comandante Faustino Pérez Hernández" of the province of Matanzas, in the period from January 1st 2017 to January 1st, 2019. The 15 patients undergoing neurosurgery were the study universe. Results: the population mean was 55 years; female sex prevailed (73.3 %). Headache was the most frequent clinical manifestation as well as meningiomas of cerebral convexity in terms of topography. The average lesion size was 4.4 cm; the meningothelial variety (40 %) was the most commonly found; grade II resection was the most used one. Post-surgical cerebral edema (26.6%) predominated. Karnofsky scale score at discharge was higher than at admission. Conclusions: the earlier the clinical diagnosis and surgical treatment are performed in patients with meningioma the better will be their quality of life at discharge (AU).

RESUMEN Los quistes epidermoides constituyen el 1 % de los tumores intracraneales y el 7 % de los del ángulo pontocerebeloso. Los colesteatomas son lesiones benignas que se originan de restos de tejidos epitelial ectodérmicos que quedan en el sistema nervioso central, al cerrarse el tubo neural entre la tercera y quinta semana de gestación. Se trata de un paciente remitido de la Consulta de Neurooftalmología con crisis de cefalea y toma de los pares craneales III, IV, V, rama oftálmica desde hace 2 semanas. Se le realizaron estudios imagenológicos donde se constató un tumor hipodenso en región silviana frontotemporal izquierdo. Se interpretó como un quiste arcnoideo. Se le aplicó un bordaje pterional transilviano con apoyo neuroendoscópico y para sorpresa del equipo quirúrgico se abordó un tumor perlado solido identificado macroscópicamente como un colesteatoma silviano. Se resecó la totalidad del tumor cerebral.


ABSTRACT Epidermal inclusion cysts constitute 1% of the intracranial tumors and 7% of the cerebellopontine angle ones.Cholesteatoma are benign lesions originated from the remains of ectodermic epithelial tissues remaining in the central nervous system when the neural tube closes between the third and fifth week of pregnancy. The case deals with a patient remitted from the Neurophthalmologic Consultation with migraine crisis and lesion on the III, IV, V cranial nerves, ophthalmologic branch, for two weeks. Image studies were carried out, showing a hypo dense tumor in the left silvian frontotemporal region. It was taken as an arachnoid cyst. A pterional trans-silvian approach with neuroendoscopic support was applied, and the surgical team was surprised when they found a solid pearly tumor that was macroscopically identified as a silvian cholesteatoma. The cerebral tumor was totally resected.

Resumen: Objetivo: determinar el efecto de los juegos en espacio reducido (JER) sobre el consumo máximo de oxígeno (VO2Máx.) en futbolistas aficionados. Método: se realizó un experimento controlado con asignación aleatoria, con grupo experimental y control. Se evaluó antes y después a 16 futbolistas de sexo masculino, edad 19,5±1,7, peso 73,6±8,5, talla 177,2±6,7 e IMC 23,2±1,6. El grupo experimental entrenó con JER, y el grupo control con método continuo variable tipo dos (MCV2). La intensidad se controló con monitores cardíacos. El VO2Máx. fue medido con el test de Léger. Se obtuvo aval del comité de ética de la Universidad de Antioquia. Los datos se evaluaron mediante análisis multivariado de la varianza (MANOVA) y estadística descriptiva. Se utilizó el paquete estadístico SAS UNIVERSITY. El nivel de significancia se estableció en (p<0,05). Resultados: no hubo diferencias significativas entre JER y MCV2 para la variable VO2Máx., ni mejoras significativas entre pretest-postest en los grupos. Conclusión: los programas de entrenamiento no mejoraron significativamente el VO2Máx.


Objective: To determine the effect of small sided games (SSGs) on maximum oxygen uptake (VO2Máx..) in amateur soccer players. Method: A controlled experiment, with random assignment in two groups (experimental and control), was conducted. Sixteen male soccer players, age: 19,5±1,7; weight: 73,6±8,5; height: 177,2±6,7; BMI: 23,2±1,6, were evaluated before and after. The experimental group trained with SSGs and the control group trained with the continuous variable method type two (CVM2). The intensity was controlled by heart rate monitors. The VO2Máx. was measured with the Leger test. Endorsement of the Ethics Committee of the Universidad de Antioquia was obtained. Data were evaluated by multivariate analysis of variance (MANOVA) and descriptive statistics. The statistical package SAS UNIVERSITY was used. The level of Statistical significance was established in (p<0.05). Results: There were not significant differences between SSGs and MCV2 for the variable VO2Máx., nor significant improvements between pretest-postest in any of the groups. Conclusion: Training programs did not significantly improve the VO2Máx.

Conclusão: os programas de treinamento não melhoraram significativamente o VO2Máx.

@#BACKGROUND: Overwhelming hemorrhage or other intra-abdominal complications may be associated with obstetrical or gynecologic (OB/GYN) procedures and may require the surgical training of an Acute Care/Trauma Surgeon. The OB Critical Assessment Team (OBCAT Alert) was developed at our institution to facilitate a multidisciplinary response to complex OB/GYN cases. We sought to review and characterize the Acute Care/Trauma Surgeon's role in these cases. METHODS: We conducted a retrospective review of all emergency consults during an OB/GYN case at our institution from 2008 to 2015. An OBCAT is a hospital based alert system designed to immediately notify OB/GYN, anesthesiology, Acute Care/Trauma, the intensive care unit (ICU), and the blood bank of a potential emergency during an OB/GYN case. RESULTS: There were 7±3 OBCAT alerts/year. Seventeen patients required Acute Care/Trauma surgery intervention for hemorrhage. Thirteen patients required damage control packing during their hospitalization. Blood loss averaged 6.8±5.5 L and patients received a total of 21±14 units during deliveries with hemorrhage. There were 17 other surgical interventions not related to hemorrhage; seven of these cases were related to adhesions or intestinal injury. Seven additional cases required evaluation post routine OB/GYN procedure; the most common reason was for severe wound complications. There were three deaths during this study period. CONCLUSION: Emergency OB/GYN cases are associated with high morbidity and may require damage control or other surgical techniques in cases of overwhelming hemorrhage. Acute Care/Trauma Surgeons have a key role in the treatment of these complex cases.

Children present with flatfoot from birth and it resolves along infancy. There have been several risk factors identified for the development of flatfoot: male sex, young age, overweight and obesity. The prevalence of flatfoot decreases with age.The aim of this study was to determine the prevalence of flatfoot in two different populations with different social, cultural and geographically characteristics in Colombia.Patients and methods: This is a cross sectional study made on school children between 3 to 10 years of age, from several schools in Bogota and Barranquilla. From 940 total children 60% were from Bogota. Flatfoot was diagnosed according to physical exam.Results: We found a global prevalence of flatfoot of 15.74%, distributed 20,8% in Bogota and 7.9% in Barranquilla. The children from 3 to 5 years had a prevalence of 30,9%, decreasing significantly after this age. It was found that children 3 to 5 years old from Bogota had a prevalence of flatfoot of 38.3% while children from Barranquilla only 17.3%, decreasing significantly in children older than 6 years. In the multivariate analysis we found an association between flatfoot with age, city, gender and body mass index.Discussion: We found a bigger prevalence of flatfoot in the population of Bogota compared to Barranquilla suggesting an influence of social, cultural and racial factors in the development of flatfoot. The diminished prevalence of flatfoot in children over 6 years of age suggest that therapeutic measures before this age are not recommended

ABSTRACT

El meningocele occipital es una malformación congénita producida por un defecto en el cierre del tubo neural. El curso natural de esta enfermedad es hacia la muerte por infección del sistema nervioso central o hacia un grado de incapacidad motora y sensitiva avanzada. Se han descrito diversas formas clínicas de esta malformación. Su diagnóstico se basa en el examen físico y estudios imagenológicos y neurofisiológicos, indispensables para conocer el contenido del saco herniario y determinar el tipo de cirugía a realizar. Se presentó el caso de un paciente que tuvo que ser intervenido quirúrgicamente, solamente por el examen físico y el estudio radiográfico simple de cráneo, debido a la imposibilidad económica de los padres de realizar el resto de los exámenes complementarios. Se realizó apertura del saco herniario, con drenaje del líquido cefalorraquídeo y reparación de los planos musculares y cutáneos. El paciente evolucionó favorablemente.


The occipital meningocele is a congenital malformation produced by a defect in the neural tube closure. This disease's natural course is toward death caused by the infection of the central nervous system or toward some level of advanced motor or sensitive disability. There have been described several forms of this malformation. Its diagnosis is based in the physical examination and in neurophysiologic and imaging studies, indispensable to know the content of the hernia sac and to determine the kind of surgery to carry out. We presented the case of a male patient that had to be surgically treated only for the results of the physical examination and simple radiographic study, due to the economical family impossibility of paying the rest of the complementary examination. We opened the hernia sac, drained the cerebrospinal fluid and repaired the skin and muscular planes. The patient had a favorable evolution.

ABSTRACT

La intoxicación por organofosforados constituye un problema de salud provincial por la magnitud del perfil agrícola de sus pobladores. Objetivo: se diseñó este estudio descriptivo para describir las características clínico - epidemiológicas de las intoxicaciones por organofosforados en el cuerpo de guardia del Hospital Clínico Quirúrgico León Cuervo Rubio en el período de enero 2006 a diciembre de 2008. Método: se realizó un estudio observacional, descriptivo y retrospectivo en el Hospital Clínico Quirúrgico León Cuervo Rubio entre el 1ro. de enero de 2006 y 31 de diciembre de 2008. Resultados: el 62 % de los pacientes fueron del sexo masculino, entre 20 y 49 años de edad y los habitantes de zonas rurales un 63.2%. La hospitalización en las unidades de terapia por la gravedad del envenenamiento fue de un 100% para los severamente graves, predominando la categoría voluntaria en relación a la intencionalidad para un 66.3 %. Conclusiones: en Pinar del Río la intoxicación por organofosforados es casi intencional y ocurre en hombres con edades laborales activas, principalmente con lesiones severas, lo que produce una elevada letalidad.


Poisoning with organophosphate compounds constitutes a health problem in Pinar del Rio province due to the agricultural traditions of its population. Objective: a descriptive study was designed to characterize the clinical-epidemiological behavior of poisoning with organophosphate compounds in the emergency room at "Leon Cuervo Rubio" General Teaching Hospital during January 2006 to December 2008. Method: an observational, descriptive and retrospective study was conducted at "Leon Cuervo Rubio" General Teaching Hospital during January 1st, 2006 to December 31st, 2008. Results: 62% of the patients were male, between 20 and 49 years old and rural zone inhabitants (63.2%). The admission in the Intensive Care Units was 100% for those severely-ill due to the severity of poisoning, the category of voluntary nature in relation to those who carried it out intentionally (66.3%) prevailed. Conclusions: in Pinar del Rio poisoning with organophosphate compounds is highly intentional and it occurs in men in active working ages; mainly severe lesions, provoking a high lethality.

ABSTRACT

El cáncer cérvico-uterino (CaCU) representa el 6% de todas las neoplasias malignas en mujeres. Evidencia epidemiológica y molecular implican al virus del papiloma humano (VPH) en la etiopatogenia del CaCU y los genotipos más frecuentemente asociados son el 16 y 18 (VPH-16 y -18). La radioterapia (RT) y la quimiorradiación (RT-QT) son las dos modalidades de tratamiento primario para el CaCU, con tasas variables de resistencia o persistencia de la enfermedad. El objetivo de este trabajo fue estudiar, por primera vez, la respuesta tumoral antes, durante y después de la RT y la RT-QT y analizar la carga viral de VPH-16 y -18 en pacientes mexicanas con CaCU. Se obtuvieron muestras de tejido tumoral en pacientes con CaCU: antes, durante y después de la RT y RT-QT, se determinó el número de copias absolutas del VPH-16 o -18 y se estudió la respuesta en relación a la carga viral por PCR en tiempo real. La disminución de la carga viral de VPH para RT-QT y RT fue de 100 y 62%, respectivamente. Se observó una mejor respuesta con la modalidad de tratamiento combinado, por lo que se sugiere que la carga viral de VPH (-16 y -18) podría ser un potencial indicador para evaluar la respuesta de ambos tratamientos en el CaCU.


Cervical cancer (CaCU) represents 6% of all malignancies in women. Molecular and epidemiological evidence relate the human papilloma virus (HPV) with the pathogenesis of the CaCU genotypes, the most frequent genotypes being 16 and 18 ( VPH-16 and -18). Radiotherapy (RT) and chemoradiotherapy (ChRT) are the two primary treatment modalities for CaCU, with variable rates of resistance or persistence of the disease. The aim of this work was to study for the first time, the tumorai response before, duríng and after RT and RT-QT and analyze HPV-16 and -18 viral load in a Mexican population with CaCU. Samples of tumor tissue were collected from patients with CaCU: before, duríng and after RT and ChRT. The absolute number of HPV-16 or -18 copies was determined, and the response related with viral load in both treatments was studíed by real-time PCR-based in fluorescence. The percentage of decrease ín HPV viral load for ChRT and RT was 100% and 62%, respectívely With ChRT, the response was better than with RTalone. HPV (-16 and -18) viral load ís suggested to be a potentíal índícator to evalúate CaCU response in both treatments.

ABSTRACT

Los lipoblastomas pertenecen al grupo de tumores de tejidos blandos de tipo benigno del grupo lipomatoso que tienen su origen en células mesenquimales y que están constituidos por adipositos en diferentes estadios de maduración, sin características malignas como pleomorfismo o anaplasia. Son relativamente raros y casi exclusivos de los niños. Se ubican generalmente en la región superficial de las extremidades generando deformidades visibles, motivo por el cual consulta la mayoría de los pacientes. El diagnóstico diferencial más importante es el liposarcoma, tumor maligno de células adiposas. El diagnóstico definitivo sólo se alcanza mediante el estudio histopatológico. Se presenta el caso clínico de un lactante de 23 meses quien presentaba una masa en el hombro derecho de un año de evolución y de aumento progresivo. El resultado del estudio de este caso clínico configuró el diagnóstico de lipoblastomatosis.
